If you would like to have this purchase shipped to a different address than your billing address, then your A Credit Card Authorization Form along with a photograph of your ID and the Payment Card is required.  The form can be downloaded here and e-mailed to info@thecavesmokeshop.com.

Credit Card Fraud is a major issue in both Online Retailers, especially in our scene.  This from protects both consumers and retailers from credit card fraudulance, and keeps criminals out of our community!
